ROLE OVERVIEW: The Hospice Activities & Life Enrichment Coordinator travels between assigned hospice accounts to support patients, families and clinical teams through meaningful activities and compassionate presence. This role enhances quality of life, strengthens team support and reinforces hospice engagement with partner facilities.

RESPONSIBILITIES:

• Provide individualized and small group activities that promote comfort, dignity and emotional well-being.
• Offer companionship, legacy activities and supportive presence to patients and families.
• Travel between hospice accounts to support liaisons with activities and events.
• Collaborate with Community Liaisons to ensure activities are being performed in high census buildings as a value to the communities that we serve.
• Maintain accurate documentation and adhere to HIPAA and hospice policies.
• Complete reports and present to business development leadership.
• Adhere to the practice of confidentiality regarding all patients, staff, customers and all aspects of the organization.
• Ensure that Grace Healthcare Services' best practices are incorporated into all job activities/responsibilities.
• Perform other duties as necessary.

QUALIFICATIONS:

• High School diploma or equivalent.
• Valid driver's license and ability to travel between accounts
• Experience working with the elderly, hospice or healthcare populations preferred.
• Strong interpersonal skills with a compassion, patient centered approach.
